Eligibility for Health Insurance
Coverage through the Insulators Local 30 Health Care Plan.
Eligibility for each quarter is based on the amount of hours worked in the previous months. To retain coverage, please see the following chart by quarter.
January 1st - Member retains coverage with 350 hours worked during September, October, and November OR with 700 hours worked in June through November.
April 1st - Member retains coverage with 350 hours worked during December, January, and February OR with 700 hours worked September through February.
July 1st - Member retains coverage with 350 hours worked during March, April, and May OR with 700 hours worked December through May.
October 1st - Member retains coverage with 350 hours worked during June, July, and August or with 700 hours worked March through August
NOTES
- Once you obtain eligibility you will remain covered for a three-month period based on hours worked as outlined above.
- Termination will occur when you fail to work these hours on either March 31, June 30, September 30, or December 31.
- If you lose eligibility you may self-pay
